Treatment and follow-up

My mandibular
advancement device.

A treatment worn in your mouth during sleep, tailored to your situation and monitored over time.

A mandibular advancement device (MAD) holds the lower jaw slightly forward to help air pass through. It may be offered for some cases of obstructive sleep apnoea, depending on the assessment.

The clinic's usual choice

Dr Christophe Chevalier mainly prescribes the ResMed Narval CC oral appliance. The choice of device and the practitioner who fits it is discussed with you, according to your situation.

What treatment options are available?

Usually prescribed appliance

Narval CC by ResMed

A custom-made oral appliance produced from dental impressions. Its two splints are joined by connecting rods; the practitioner fitting the device gradually adjusts the degree of advancement.

It is worn during sleep and works without a machine or mask.

Depending on your situation

Other oral appliances or CPAP

Other oral appliances may be discussed if the characteristics of your mouth, comfort or treatment requirements justify this.

Continuous positive airway pressure (CPAP) remains another option. The choice depends in particular on the severity of the disorder, associated medical conditions and whether dental treatment is feasible.

A protective mouthguard for tooth grinding is not a mandibular advancement device for apnoea. Before buying an anti-snoring device, discuss whether you need a sleep assessment.

Could it be suitable in my case?

Prescribing a device involves two complementary assessments: your sleep assessment and an examination of your teeth and jaw. An oral appliance is not suitable in every situation.

Points to check

  • The condition and stability of your teeth and gums.
  • Whether the appliance can be held securely in place.
  • Jaw movement and the condition of the jaw joints.
  • Any ongoing or planned dental or orthodontic treatment.

What may prevent or delay treatment

Significant gum disease, very loose teeth, insufficient retention or certain painful conditions and diseases of the jaw joints may limit the use of an appliance or make it unsuitable.

In other cases, preliminary dental treatment or an additional opinion may allow the suitability of an appliance to be reassessed.

This page concerns the Narval appliance intended for adults. Treatment options for children or adolescents require a specific assessment; they cannot be inferred from the adult care pathway.

A contraindication to an oral appliance does not mean there is no treatment: we discuss a suitable option together. This list does not replace an examination or the contraindications in the device's instructions for use.

From prescription to review.

  1. Deciding on treatment. Dr Chevalier assesses whether the treatment is appropriate and refers you to a trusted practitioner for assessment and fitting.
  2. Examination and manufacture. The practitioner checks feasibility, takes impressions and orders the custom-made appliance.
  3. Gradual adjustment. Fitting and adjustment aim to balance comfort and effectiveness. Do not adjust the degree of advancement yourself without instructions.
  4. Testing while wearing the appliance. Respiratory polygraphy or polysomnography may be arranged while you wear the appliance to check its effectiveness. Snoring less or feeling better is not enough to confirm that the apnoea has been corrected.
  5. Continuing follow-up. Sleep follow-up and dental monitoring remain necessary, particularly if your weight, symptoms or teeth change.

Living with my oral appliance.

How should I wear it and report a problem?

Follow the wearing instructions given when the appliance is fitted. Report persistent pain, joint discomfort, a change in how your teeth meet, a poor fit or breakage to the practitioner who adjusts your appliance. If your sleep symptoms persist or return, also contact the clinic.

How should I care for my Narval?

After use, clean it with lukewarm drinking water and a dedicated brush, rinse it, dry it and put it away in its case. ResMed also recommends thorough cleaning twice a week with a compatible product.

Avoid hot water, toothpaste, mouthwash, soap and household cleaning products. Follow your appliance's instructions for use and the manufacturer's instructions.
